Manage a diverse caseload of individuals with disabilities, to assist those individuals in accessing funding, programs, and resources, with the goals of enhancing their quality of life, and increasing integration into their communityKey ResponsibilitiesPlanning SupportsEstablish a trusting relationship with the individual being served and their teamIdentify the needs of the person being supportedCollaborate with the individuals team to develop an Individualized Service Plan that will support the person in meeting identified needsLinking ResourcesResearch community resources and funding programsLink individuals to needed resources, through providing information, making referrals, and coordinating meetingsQuality EnhancementMeet with individuals and their planning team, following state and federal frequency requirementsMonitor and assess the quality, timeliness, and effectiveness of services and supports throughout the individuals plan year data= Recommended Skills Certified Nurse Practitioner Curiosity Estimated Salary: $20 to $28 per hour based on qualifications.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
